SPECIAL POPULATIONS

Cabarrus Rowan Community Health Centers, Inc. was originally founded in 2006 to provide comprehensive, high-quality primary health care to all the residents of Cabarrus and Rowan Counties of NC, including low-income, indigent, and uninsured patients who may not otherwise be able to afford health care via traditional sources. CRCHC offers a wide range of health services including Primary Care/Adult Medicine, Oral Health, Behavioral Health, Telehealth, Covid-19 Services, Mobile Services, and Special Population Services (migrant seasonal workers, public housing, and homeless population).

​Through a network of eight locations and a mobile unit throughout Cabarrus and Rowan counties; we are growing and seeking new ways to serve more. Our network of health centers is committed to serving and strengthening our community. We intend to provide each person with a system of comprehensive health care that promotes wellness. In 2021, CRCHC provided services to over 15,313 patients from over 50 zip codes across NC with 21% (3,218) coming out of our special population programs which include Migrant Health 63, Homeless Health 130, and Public Housing 3,025.

Migrant Health Program

CRCHC entered a partnership to provide care to migrant farmworkers through a relationship with Patterson Farms located in China Grove, NC in 2019. This partnership led to a clinic being opened at Patterson Farms in 2019. To date CRCHC has provided services to over 500 Migrant Farm Workers since it’s program inception.

Our migrant health clinic located at Patterson Farms in China Grove, NC along with our mobile medical and dental unit provide multiple farms with access to care seasonally. In addition, year-round agricultural workers may be seen at any of our sites. Our providers can refer to nearby hospitals or one of our CRCHC locations, if necessary, when treated at our medical unit. Our program is equipped to provide the following services to our migrant population.

Homeless Health Program

CRCHC has a long history with providing services to our county homeless population through our health clinics located throughout Cabarrus and Rowan counties in NC. In 2019 CRCHC entered a partnership to provide care to the homeless population directly at a homeless shelter in Rowan County through a relationship with Rowan Helping Ministries located in Salisbury, NC in 2019. To date CRCHC has provided services to over 800 homeless individuals since it’s program inception.

Our Homeless health services clinics located inside Rowan Helping Ministries located in Salisbury, NC along with our mobile medical and dental unit provide access to care year-round for the homeless population in Salisbury, Nc. In addition, year-round homeless individuals may be seen at any of our sites. Our providers can refer to nearby hospitals or one of our CRCHC locations, if necessary, when treated at our medical unit. Our program is equipped to provide the following services to our homeless population.

Homeless staff are bilingual in the Spanish and English language, communicating with patients in the tongue they understand and feel comfortable speaking. With 5 sites to refer for follow-up CRCHC is dedicated to follow-up with care management for our agriculture workers.
CRCHC Homeless health services is pleased to be part of the Healthcare For The Homeless movement striving to reach the goal of provide quality medical, dental and behavioral health services to this target population.

Public Housing Health Program

CRCHC entered a partnership to provide care to the Logan Housing Authority community in the City of Concord, NC in 2009. This partnership led to a clinic being opened at Logan Housing Authority in 2009. We continue to serve our public housing community at the Logan Housing Authority and to date CRCHC has provided services to over 20,000 public housing residents since it’s program inception.

Our public housing clinic is in the Logan Housing community in Concord, NC and provides services all year round. In addition, public housing resident may be seen at our network of clinics. Our providers can refer to nearby hospitals or one of our CRCHC locations, if necessary, when treated at our medical unit. Our program is equipped to provide the following services to our migrant population.
