Cabarrus Rowan Community Health Centers and Opportunity House have announced that Opportunity House will officially join the CRCHC network effective July 1 through a strategic integration designed to preserve, strengthen, and expand services for individuals and families experiencing homelessness and housing instability in Concord and Cabarrus County.
CRCHC is a federally qualified health center committed to delivering high-quality, affordable, and comprehensive healthcare to underserved communities. Serving more than 19,000 patients across Cabarrus, Rowan, and Mecklenburg counties, CRCHC provides integrated medical, behavioral health, and oral health services through a growing network of community-based health centers.
Through this integration, Opportunity House will continue its mission of supporting individuals experiencing homelessness and transitional housing needs while gaining expanded access to CRCHC’s clinical services, healthcare infrastructure, care coordination, operational support, and community partnerships. The goal is to build a more coordinated, whole-person model of care that addresses both immediate health needs and the social drivers of health that impact long-term stability.
As part of the transition, CRCHC plans to establish standing medical and behavioral health services on-site at Opportunity House. This expansion will create a comprehensive community resource hub designed to provide primary care, behavioral health support, preventive care, care coordination, outreach services, social service navigation, and referrals to additional community resources in one location.
This model is intended to reduce barriers to care by bringing services directly to individuals in a familiar and accessible environment. It also strengthens the connection between healthcare access, housing support, mental health services, substance use support, and pathways toward long-term self-sufficiency.
Since 2021, CRCHC has provided care to Opportunity House clients through its mobile health unit, bringing essential services directly to individuals facing barriers to traditional healthcare access. The July 1 integration builds upon this partnership by transitioning from mobile outreach to permanent, coordinated on-site services.
“This transition is about preserving the heart of Opportunity House while expanding its reach and impact,” said Don Holloman, MEd., CHCEF, CEO of CRCHC. “By bringing Opportunity House into the CRCHC network, we are strengthening the safety net for individuals experiencing homelessness and housing instability while expanding access to medical care, behavioral health, oral health, and wraparound support services.”
He added that the integration reflects a broader vision for community-centered care. “This partnership allows us to address not only healthcare needs, but also the social determinants of health that directly impact long-term stability, wellness, and quality of life. Our vision is to create a true community hub where individuals can access housing support, healthcare, behavioral health services, outreach resources, and care coordination all in one location.”
Kimani Varner, Executive Director of Opportunity House, emphasized the impact the partnership will have on the organization’s mission and the individuals it serves.
“Joining the CRCHC network allows Opportunity House to continue its mission with greater resources, stronger infrastructure, and expanded access to care for the people we serve,” said Varner. “This partnership ensures that our clients will have access not only to housing support, but also to healthcare, behavioral health services, and additional community resources that support long-term stability.”
Varner added, “The ability to integrate standing medical and behavioral health services directly within Opportunity House will have a transformational impact on the people we serve. This integration positions us to reach more individuals throughout Concord and Cabarrus County while preserving the heart of the Opportunity House mission.”
The July 1 transition is expected to enhance services across Concord and Cabarrus County by preserving a vital community organization, supporting staff continuity, expanding healthcare access, and creating a more comprehensive response to homelessness, housing instability, and behavioral health needs.
CRCHC and Opportunity House will continue working closely with staff, community partners, donors, and stakeholders throughout the transition to ensure continuity of services and a smooth integration.
